What do you need for your appointment

Please remember to:

  • Bring your valid Medicare | DVA | Seniors Card

  • Bring a list of any medications you are taking

  • New patients please print and fill in our new patient form – New patient form or;

  • Arrive 10 minutes prior to your appointment to fill out the necessary paperwork.

We’ll make every effort to book you in for an appointment with your preferred doctor and time. If we can’t offer you your first choice, we’ll find the next best appointment available. If you’re unable to attend your appointment, please contact us and give as much notice as possible.

Gisborne Medical Clinic

The majority of our GP’s have undertaken additional specialist training and have been awarded with The Fellowship of the Royal Australian College of General Practitioners (FRACGP). This is a specialist general practice qualification accredited by the Australian Medical Council.

Majority of our Standard GP consultations are bulk-billed for patients who

hold a Medicare card.

 

Some consultations may incur a fee including:

  • Superannuation Paperwork

  • Pre Employment Medical

  • Commercial/Heavy Vehicle Licences

  • Procedures

  • Iron infusion

Please check with reception regarding the fees.
